Tobacco and nicotine
Vapes, heated tobacco and nicotine pouches count the same as cigarettes.
Does anyone smoke indoors where you live?
Breathing someone else's smoke at home lowers the measure as much as the habit itself.
Vegetables and fruit — at least 5 servings a day?
A serving is about a handful or one medium fruit.
Fish or seafood — twice a week or more?
Any fish counts, fresh or frozen.
Whole grains every day — oats, buckwheat, brown rice, wholemeal bread?
White bread, semolina and white pasta do not count.
Sugary drinks — none, or no more than one glass a week?
Juices, soft drinks, energy drinks, coffee with syrup.
Processed meat — no more than twice a week?
Sausage, hot dogs, ham, bacon.
Blood pressure
If you do not know, say so. That is a normal answer, and it is informative too.
Cholesterol
From your latest lipid panel. The index uses non-HDL cholesterol: total minus HDL.
Blood sugar
Fasting glucose or HbA1c from your latest test.

Spinach, lettuce, rocket, herbs — 7 servings a week or more.
Other vegetables — 2 servings a day or more?

Berries — twice a week or more?
Fresh or frozen; a serving is about 100 g.
Fruit other than berries — every day?
One serving a day or more.
Red meat, bacon, sausage — no more than 3 times a week?
